SLIDING FEE SCALE SCHEDULE
Aligned Behavioral Health LLC operates a sliding fee schedule aligned with the U.S. government's federal poverty guidelines to enhance the affordability of healthcare services. Our commitment ensures that financial constraints do not impede patients from accessing vital care. Eligibility for our Sliding Fee Discount Program is determined by assessing household size and annual income. To apply, individuals must complete an application and provide proof of income. Discounts are granted in accordance with the federal poverty income guidelines, which are updated annually by the federal government. While fees for eligible services are reduced, patients are responsible for a nominal fee for the service received, payable at the time of service. Annually, both existing and new patients are required to provide proof of income*, a valid photo ID, and current income tax forms. This process helps maintain accurate eligibility records and ensures the continued provision of affordable care to those in need. *Paystubs = 1 month | Tax returns = 1 year | Proof of no income = 3 months

Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay a $75 no-show fee.
